Are you aware that a Medical Advance Directive and POLST (Physicians Orders for Life Sustaining Treatment) forms can ensure your medical preferences are respected and followed by your family, provider and care team? Learn the important of each, and when and how you need to complete them. An Advance Directive and a POLST form will help your loved ones know your wishes and ensure emergency medical services and other medical providers follow important decisions during a health crisis. Understanding and properly completing an Advance Directive is the first step towards getting your plans in order. Register in advance; Space is limited.
