The Office on Aging and Independence will be presenting a table for Health Care Decision Day. This effort is led by the Guardianship Program and designed to encourage people to ease the burden of decision-making for their loved ones as they make out Advance Directives and plan for their futures. We will be providing MD Advance Directive forms, Five Wishes packets, lists of lawyers, and other handouts to aid in the process. It is never too early to plan for your peace of mind!
