RELEASE AND WAIVER OF LIABILITY
This is the 2026 player waiverThis waiver must be completed by parent/guardians for all participants participating in the 2026 3on3 Basketball Tournament
Tournament Date: August 15th (Rain Date: August 16th)

In consideration for my participation in The Community House, Hinsdale Community 3-on-3 Basketball Tournament (hereinafter the “Event”), I hereby, for myself, my heirs, executors, insurers, successors and administrators assume any and all risks associated with my participation in the event, and unconditionally and voluntarily release and waive any and all rights and claims that I may now, or in the future, have against The Community House any affiliated entities, all event sponsors, all event organizers, all event workers/contractors, employees, board of trustees, officers or agents (hereinafter referred to as the “Releasees”). The Releasees are hereby released from any and all liability and claims for injuries, death, damage or loss which I may have or which may accrue to me from my participation in the event and the risk of loss of personal property by theft or otherwise. I further agree to indemnify and hold harmless and defend the Releasees from any and all claims, damages, liabilities and/or costs (including attorneys' fees) arising out of, connected with or in any way associated with my participation in the event.

This document is intended to be as broad and inclusive as permitted by law, and if any portion hereof is held invalid, it is agreed that the balance shall notwithstanding, continue in full legal force and effect. This document embodies the entire agreement with respect to these matters and supersedes any previous or contemporaneous negotiations or agreements. Each of the undersigned agrees that no promise or inducement has been offered except as herein set forth, and that this document has not been executed based upon statements or representations by any person or entity other that those set forth herein. This document shall be governed by and construed under the laws of the state of Illinois without regard to conflicts of law principles therein.

Further, I hereby grant full permission for event organizers to record any or all of my participation in this event for photos, motion pictures, TV, radio, recording, videotapes, and other media known or unknown, and to use them, no matter by whom taken, in any manner for publicity, promotions, advertising, trade or commercial purposes, without any reimbursement of any kind due to me, or the need to pay me any fee.
