DPHD will provide Flu, COVID (if available), Shingles, and Pneumonia vaccines. For clinic information call 740-203-2040 or visit DelawareHealth.org.

• Click submit at the end of the form and check your email for confirmation.

Sign-ups will close a week prior to the clinic for vaccine ordering. DPHD cannot accept Medicare Supplemental insurances or HMO plans (except Anthem HMO).

Cancellation Policy: You will receive a refund for fee-based activities canceled by SourcePoint or for activities from which you withdraw before the deadline. Refunds are not available for activities you are unable to attend.
