This Evidence-based chronic disease self-management program is designed for people and/or their caregivers living with chronic health conditions like high blood pressure, diabetes, obesity and more. Each week, small groups meet to discuss topics like nutrition, exercise, communication with family members and health care providers, feelings of frustration, and more. This program meets for 2.5 hours for six consecutive weeks.
