Description
Join us for an information session with the Loma Linda University Health PACE (Program of all inclusive care for the Elderly). PACE provides whole-person support for seniors, from medical care and rehabilitation to nutrition support, social services, in-home services and transportation. Free snacks are provided.
To qualify for LLU's PACE program, you must
- Be at least 55 years old
- Meet nursing home eligibility criteria
- Live in the Loma Linda University Health PACE service area (serving San Bernardino and Riverside counties)
- Be able to live safely in the community.
