Additional Information
A parent or legal guardian MUST submit the following items to the registrar on or before Tuesday July 6, 2010 in order to complete the registration process and receive the $50 in raffle tickets:
1. Completed and signed City of Tamarac Waiver
2. Copy of Birth Certificate for the League Rep to keep
3. 2010 Physical Form (Due before first practice on Monday July 13, 2010
4. Copy of Utility Bill for Proof of city of Tamarac Residency or Copy of "Out of City Card" for the League to keep (May be purchased at the Tamaract Community Cener at 8601 W Commercial Blvd for $50)
PLEASE NOTE: Registration WILL NOT be completed and your child WILL NOT be allowed on the field for the first practice on Tuesday July 6, 2010 without ALL of the above completed.
