ATTN TEAMS AND INDIVIDUALS
To become a fundraiser and join the fight against melanoma click "Become a Fundraiser" at the top of this page and you will be guided to personalize a fundraising page to circulate to your friends, family and associates. Please be sure to fully complete the form so we can cheer you on.
Once your page is set up, email the webpage link you created to your contacts and ask them to support you by donating through your personal fundraising page. All of the donations are tax deductable and will go directly to research and awareness programs.
Checks and cash collected should be sent to Jack’s Fund PO Box 436 Glen Ellyn IL 60138. Please include your name and your team name (if applicable) with the donations.
