The CENTERVILLE NOON OPTIMIST SOF-T-BALL ASSOCIATION is intended to provide organized T-Ball for 4-1/2 to 7 year old boys and girls in the Centerville and Washington Township communities. Children will be taught a basic knowledge of the game and gain a rewarding and fun experience through learning to play fairly, abiding by the rules of sportsmanship. 1. Children should be born between June 1, 2003 and October 1, 2006 and should be residents of Centerville or Washington Township.

2. One game will be played (NO DOUBLEHEADERS) on Sunday afternoons at 1:00 & 2:30, beginning on May 1 and ending June 26, excluding May 29. If less than six games are played, then one make-up will be scheduled for July 3. Each teams’ individual coaches will arrange practice days and times.

3. Teams will be formed geographically. A balance of boys and girls of similar age will be a goal. Maximum number of team members is 15.

4. If you wish to have your child play on a team with certain other children, or to play for a specific coach, please make such a request on this registration form when it is submitted. The teams will be arranged with this request in mind, although there can be no guarantee that all requests can be satisfied. All siblings, as well as fathers who coach, will be put on the same team unless otherwise requested.

5. The registration fee is $45.00, plus $25.00 for each additional sibling, which will cover the cost of new equipment, uniforms, trophies, and other expenses. NO REFUNDS WILL BE GIVEN after March 20, 2011.

6. The success of this program is based on parent participation. If you will serve as Coach or Assistant Coach, please note it on the registration form.

7. Your child’s coach will be contacting you in mid April with information about the league, your team, and your schedule.
